The obverse of the medal features a portrait profile of John Cardinal OConnor in his stately attire. The inscription JOHN CARDINAL OCONNOR is centered along the right side of the medal and the inscription ARCHBISHOP OF NEW YORK is centered along the left side of the medal. The reverse of the medal features the Seal of the Archbishop of New York in the center of the medal. The inscription, PRIEST CHAPLAIN HUMANITARIAN is centered along the top of the medal and the inscription ARCHBISHOP OF NEW YORK centered along the bottom of the medal.
